TUFTS HEALTH PLAN HMO: Patient needs to obtain a referral from their Primary Care Physician (PCP).
BLUE CROSS/BLUE SHIELD HMO BLUE AND BLUE CHOICE: Patient needs to obtain a referral from their PCP.
HARVARD PILGRAM HEALTH CARE: Patient needs to obtain a referral from their PCP.
MEDICARE: Patient needs to bring in a written prescription from their PCP or Specialist.
